Job Description

At Summerset we are here to bring the best of life to our residents. To do this, we know we need to take care of our people to form a team who are strong enough to care, operate as one team, and strive to do their best. 

The Care Coordinator plays a pivotal role in ensuring the highest quality of care and support for residents. Responsibilities include coordinating Caregiver activities effectively across each day, escalating resident care needs to Nurses on shift and Care Centre Managers and ensuring access to essential resources for the delivery of person-centered care. 

This is a new permanent vacancy that would involve working across a mixture of rotational shifts morning, afternoon and nights.

Qualifications

To be considered a suitable candidate for our role you will bring the following: 

  • Minimum Level 3 National Certificate in Health, Disability and Aged Support or Summerset recognised equivalent.  
  • Demonstrate strong leadership and communication skills for effective coordination and guidance of Caregiving staff.  
  • Handle complex situations, make sound decisions under pressure, and show empathy, kindness, and patience in challenging situations 
  • Be available to work on a rostered and rotating shift pattern (including mornings, afternoon and weekends).
